Required Documentation

Send an enlarged photo copy of both sides of your driver’s license along with the following forms found under the Files section on this page:

College Employees:

  • Print and complete: Penn-Dot DL-503, sections C & E only
  • Print and complete and have your supervisor sign the: Driver Information Form
  • Review Insurance Information for Employee Drivers of College Vehicles
  • Print, read and sign the Vehicle Transportation Policy. You are responsible to know the College Policy.

NOTE: Please use the form entitled Insurance Information for College Employee Drivers for College employees who elect to drive their own vehicle on College business or are acting on behalf of the College, as well as employees who are driving a College-owned vehicle. 

Students:

  • Print, read and sign the Vehicle Transportation Policy. You are responsible to know the College Policy.

Send the completed forms and the photo copy of your license to Rachel Sauer-Koeffler, Administrative Assistant for General Services and Facilities, Room 1 of the Safety & Facilities Building via campus mail; or scan and e-mail the documents to Rachel.Koeffler@cedarcrest.edu.

Van Certification

To schedule a 12-passenger van training session, please contact Emeritus Transportation Director, Walter Baransky at wjbarans@cedarcrest.edu.  The training session takes approximately 2 hours and includes a vehicle road test. 

Van certification candidates should download the Required Reading Materials for Van Certification found under the Files section to prepare for the training session.

Van certification candidates must also send the completed driver forms and a photo copy of their driver’s license to Rachel Sauer-Koeffler, Administrative Assistant for General Services and Facilities, Room 1 of the Safety & Facilities Building via campus mail or scan and e-mail the documents to Rachel.Koeffler@cedarcrest.edu before the training session is scheduled.
